Pediatric PICC Line Checklist/Protocol Please complete this checklist for every PICC candidate: o Fax completed Radiology request to 66521 o Call 3 east procedure nurse to schedule procedure 73208 o PICC nurse will assess pt. to determine PICC candidacy and evaluate need for sedation/analgesia with or without anesthesia involvement. o ORDER no blood draws or IV starts to antecubital fossa o EMLA cream applied to antecubital fossa 1 hour before procedure o 1 vial lidocaine at bedside If anesthesia required: o NPO order o Fax anesthesia request (to ensure pt is on the schedule, please fax request prior to 1100 day before) Post procedure: o Fax Chest XRAY request to confirm placement, specify MIDLINE or PICC LINE on request
1. No transfer can occur without contacting the Transfer Center x50909. 2. The PICU Attending is called only on certain types of transfers, as follows: a. ER to ER: ordinarily, the Transfer Center will not call PICU Attending for ER to ER transfers, but may call to discuss the case. b. Transfer of patients known to and/or accepted by a Pediatric or Surgical service going to the Pediatric floor: Transfer Center will not involve the PICU Attending, but will speak directly with the accepting service. c. Transfer of a patient not known to and/or accepted by another service: Transfer Center will call the PICU Attending who will attempt to determine if the patient needs a PICU or floor bed. If the patient is going to the floor, the Transfer Center will notify the appropriate service about the patient. d. Transfer of a patient not known to and/or accepted by another service, who requires PICU admission: PICU Attending will arrange transport and discuss the case with the referring physician at outside facility. e. Transfer of a patient, for admission to the NICU and requiring transport needs to be cleared by contacting the NICU attending. No admission to the NICU should be planned without an OK by the NICU attending. NOTE: Most floor admissions will not require Peds Transport Team. The PICU Attending is available to discuss all transports and to assist with facilitating any transfer. However, communication regarding those transfers going to the floor should include the accepting Attending physician.
